Polite formal questions use Sie. “sich anmelden” is reflexive: Ich möchte mich anmelden. “ausfüllen” separates in the present but forms “ausgefüllt” here.
These forms support registration and document appointments without pretending to cover legal advice.
“Welche Unterlagen brauchen Sie?” asks the visitor what documents they need; it does not state your appointment.
